Do GLP-1 Drugs Have Effects Beyond Weight Loss?

·

GLP-1 drugs, such as Ozempic and Wegovy, mimic glucagon-like peptide-1, a hormone released by the intestine after eating. This hormone helps coordinate the body’s response to food: it prompts the pancreas to release insulin when blood sugar rises, reduces signals that cause the liver to release stored sugar, slows the passage of food out of the stomach, and acts on the brain to reduce hunger and increase fullness. The drugs last much longer in the body than the natural hormone, allowing many to be given as a once-weekly injection. Originally developed to treat type 2 diabetes, they also produce substantial weight loss by helping people eat less. Their benefits for the heart, kidneys, and possibly some inflammatory conditions have prompted research into uses far beyond their original purpose.

There is convincing evidence for several benefits beyond glucose lowering, but much less evidence that GLP-1 drugs act as broadly effective treatments for inflammatory diseases. The important distinction is between improving a disease in someone with obesity and demonstrating an effect independent of weight loss. The evidence has advanced during 2026, particularly in psoriasis and psoriatic arthritis.

Semaglutide clearly lowers inflammatory markers. In the randomized STEP trials, CRP fell approximately 39-48% more than with placebo. Experimental work also supports effects on inflammatory signaling, including a brain-immune pathway demonstrated in mice. These findings establish biological plausibility; a lower CRP does not establish control of an autoimmune disease.

The clinical evidence varies substantially:

DiseaseEvidenceWhat it establishes
PsoriasisA 2026 randomized trial of 274 overweight or obese patients compared ixekizumab plus tirzepatide with ixekizumab alone. Complete skin clearance at 36 weeks occurred in 40.6% versus 29.0%.Credible evidence of additional benefit, but the trial was open-label and cannot distinguish weight-loss effects from direct immunomodulation. It does not establish tirzepatide as effective monotherapy. JAMA Network
Psoriatic arthritisThe randomized TOGETHER-PsA trial also favored adding tirzepatide to ixekizumab. Manufacturer-reported 52-week results showed ACR50 responses in 43.7% versus 15.7%.Promising randomized evidence for adjunctive treatment in overweight/obese patients. The 52-week figures were still awaiting detailed publication in the announcement reviewed. Eli Lilly and Company
Rheumatoid arthritisA retrospective study compared 173 patients taking semaglutide or tirzepatide with 42 prescribed these drugs who did not take them. Disease activity and pain improved more among users.Hypothesis-generating. Treatment selection was nonrandom, and disease activity was reconstructed from clinical notes rather than consistently measured with standard composite scores. Wiley Online Library
Crohn’s disease/ulcerative colitisObservational cohorts suggest fewer adverse outcomes. An Israeli study of patients with IBD and diabetes found an adjusted hazard ratio of 0.74 for a composite of adverse clinical outcomes.Encouraging association, but insufficient to establish efficacy as IBD treatment. Residual confounding remains a substantial concern. Oxford Academic

Tirzepatide is a dual GIP/GLP-1 agonist, so its results cannot automatically be attributed solely to GLP-1 activity.

Other diseases: stronger evidence, usually in a metabolic setting

  • Cardiovascular disease: SELECT randomized 17,604 overweight or obese patients with established cardiovascular disease without diabetes. Semaglutide reduced cardiovascular death, nonfatal MI, or nonfatal stroke from 8.0% to 6.5%, a 20% relative reduction. This establishes benefit beyond treating hyperglycemia, although it does not isolate an anti-inflammatory mechanism.
  • Obesity- associated Heart failure with preserved ejection fraction: In SUMMIT, tirzepatide reduced cardiovascular death or worsening heart failure from 15.3% to 9.9%, largely through fewer worsening-heart-failure events. Symptoms and physical function also improved.
  • Chronic kidney disease with type 2 diabetes: FLOW showed a 24% reduction in the primary composite of major kidney outcomes or cardiovascular death. This is meaningful organ protection, but does not establish treatment of inflammatory nephritis.
  • Knee osteoarthritis: In STEP 9, 407 obese patients received semaglutide or placebo. WOMAC pain scores fell 41.7 versus 27.5 points, while weight fell 13.7% versus 3.2%. The symptomatic benefit is convincing; cartilage preservation or an independent anti-inflammatory effect was not demonstrated.
  • Obstructive sleep apnea: Two randomized trials showed approximately 20-24 fewer apnea/hypopnea events per hour beyond placebo with tirzepatide in patients with obesity. Again, the trials establish benefit without separating it from weight reduction.

The neurological and addiction results are particularly interesting

For Parkinson’s disease, a 156-patient phase 2 lixisenatide trial showed a modest motor-score benefit. However, the larger, longer phase 3 exenatide trial was negative. Disease modification therefore remains unproved.

For Alzheimer’s disease, the two phase 3 EVOKE trials, involving 3,808 patients, found that oral semaglutide did not slow clinical progression over two years. Favorable observational associations had not predicted this result. These trials address treatment of symptomatic disease, not whether earlier exposure might prevent it.

For alcohol-use disorder, evidence is more encouraging. A 2026 randomized trial of 108 patients with obesity found a 13.7-percentage-point greater reduction in heavy-drinking days with semaglutide over 26 weeks. Smaller trials have produced mixed results across endpoints. This is a credible therapeutic signal, still requiring larger and longer confirmation.

These drugs have established benefits beyond glycemic control, and the new psoriatic-disease trials provide real evidence of an adjunctive role in inflammatory disease. The much broader claim-that they can treat inflammation independently of obesity or metabolic dysfunction-remains substantially ahead of the clinical evidence. A convincing demonstration would require disease-specific trials with weight-matched comparators or patients without obesity, measuring remission, tissue damage, or steroid requirements rather than CRP alone.
